Vehicle: 2021 Ford Mustang Mach-E Component: Image Processing Module A (IPMA) / Front Camera / Forward Collision Avoidance System The IPMA and front camera system have failed, disabling Pre-Collision Assist, AEB, forward collision warning, and adaptive cruise control while driving. The vehicle is available for inspection upon request. Warning messages "Front Camera Fault Service Required" and "Pre-Collision Assist Not Available" first appeared August 24, 2022 and have recurred consistently over nearly four years. No accident, physical damage, or obstruction was present at any occurrence. This failure puts safety at risk by silently disabling AEB and forward collision warning during normal driving — systems a driver may depend on in an emergency braking situation with no advance notice they are unavailable. The problem was confirmed by a Ford dealership in February 2026. Their repair order documents: "RAN IPMA SELF TEST AND FOUND DTC SET U3000-49... HARDWARE TEST FAILED... FOUND TSB RELATED REC REPLACE MODULE." The dealer contacted Ford, who authorized 50% off the module replacement only, characterizing it as a "loyal customer discount" rather than a warranty or goodwill repair. I declined to pay for the fix. NHTSA Campaign 11028164 (superseding TSB 25-2156) covers this exact failure. Ford's own TSB acknowledges the defect and the dealer confirmed it applies to my vehicle, yet owners are being quoted up to $8,000 for a known safety system failure. I'm requesting NHTSA investigate whether this warrants a recall. After latest software updates, the Mustang MachE gets stuck in the guest profile regularly. These profiles are responsible for controlling driver aids, one pedal driving, etc. When stuck, my vehicle removes one pedal driving and the car moves unexpectedly. One pedal driving holds with no break, this is removed and the car creeps when break removed, this nearly caused the car to crash through the garage. This could injure people in parking lots, etc. There are many other driver aids controlled by the profile which could unexpectedly get disabled. I have personally figured out how to reset and recognize this after nearly crashing the car into the garage, but Ford has issued no communications to their customers on how to recognize and revert when this issue occurs. When manufactures issue over the air updates, the need to be responsible for communicating issues and workarounds to their customers directly. The dealers (any my dealer) have been useless about any software bug and just state, wait for ford.

Second time this has happened. A message will pop up saying “please stop safely now” once you stop you can not get it out of park. If your driving you are stuck wherever you are. I was stranded the first time with no heat in 20 degree weather. They fixed it after having it for a month then it just happened again today in 80 degree weather with my kids in the car. This has happened to multiple other people as well.

I have been attempting to get two recalls done since November of 2021: NHTSA recall Numbers 21V711 and 21V712. I have called several ford dealers (O'Meara Ford, Larry Miller Ford Lakewood, Groove Ford and Schomp Ford) and no one will either schedule the repair or they will schedule it and then cancel it on the day of the incident. I've been told either ford won't pay enough to get the recall done (Approx. $350 for labor and not being clear on covering the cost of a broken panoramic roof) so the glass subcontractors won't take the job due to liability concerns or that the parts needed, (spare panoramic roof and windshield) in case the glass breaks during removal, have been on backorder so they can't (and won't order) the parts. I have become extremely concerned as the recall notice notes: "INADEQUATE URETHANE ADHESION OF THE PANORAMIC ROOF GLASS MAY RESULT IN INCREASED WIND NOISE, WATER LEAKS, AND OR THE ROOF GLASS MAY LOOSEN AND BECOME DETACHED FROM THE VEHICLE, CREATING A ROAD HAZARD AND INCREASING THE RISK OF A CRASH." So now I'm driving a vehicle for six months where my roof could come off, creating a potential hazard for me and the public driving behind, not to mention the potential damage from leaks could cause to the interior of my vehicle. It seems that neither Ford nor the dealers are taking this issue seriously in the Denver Metro area.

Ford is unable to perform two safety recalls. The first is over the windshield which can lead to its failure while driving. The second is for the glass roof which can cause a failure while driving. The local dealer in Springfield Illinois says they can not do the work and can not identify anyone locally who will do the work. The dealership with the Ford autobody repair shop in the next town over in taylorville Illinois says they can not do the work. I called Ford customer service and they just sent me to the taylorville shop that said they can't do the work. The windshield or roof can come off and no one will do the repair including Fords own autobody shop. Help.

For example I was driving in cruise control at 38 mph, when the car suddenly sped to 50mph. Or when on a highway, driving at 55mph, the car suddenly accelerated to 70mph. Started about 3 weeks ago.

Ford had a recall on the brakes. The brakes were services 3 times in relational to this issue. Now again the front end shutters when braking especially at highway speed. The dealer says the pads need to be replaced and the rotors resurfaced.This is the 4th time in 42,000 miles. they will not warranty this on going issue. If the pads need to be replaced 4 time in 42,000 mile on a vehicle with regenerative braking there is a obvious issue. I feel this is an on going issue that was never corrected.

The contact stated that while driving at an undisclosed speed and the brake pedal was depressed, the vehicle did not slow down as intended, and crashed into another vehicle. The contact sustained neck, head, and arm injuries. Medical attention was needed. A police report was filed. The vehicle was towed to a body shop. The manufacturer was not notified of the failure. The failure mileage was approximately 46,140.

A KNOWN faulty camera problem for 100s of Ford EVs. Starts off as very intermediate warnings until it stops working all together. This problem doesn't allow you to use cruise control and a host of other standard safety features. This should be a recall, I can't imagine someone getting hurt or worse because ford isn't getting ahead of this thing.

The instrument cluster displays several warning lamps and text messages at vehicle startup. The dealer is recommending a replacement of the brake booster. This issue matches a recall with the same make, model, and year car, but my VIN is not associated with any unrepaired recalls.

I bought a certified pre-owned 2021 Ford Mustang Mach-E GT. The brakes always kind of made a grinding but i figured it was the brake dust on the rotor since the car had sat on the lot for a while. I took the car into Maplecrest Ford in Union, NJ for this issue and they replaced the rear brakes under warranty. Shortly after that I felt a vibrating on the highway, I figured it could be the brakes bedding in. After that it became more persistent and I took the car to the dealer on [XXX] and they told me the front brakes had to be replaced. The car was purchased with 16,500 miles, went to the dealer at 17,000 miles and has 17,500 miles right now. It is impossible to believe that brakes on an EV (where most braking is regenerative) need to be replaced down to the caliper. I read Ford TSB 22-2023: Vibration/Pulsation From Brakes - Brake Pad Material Stuck To Rotor - Built On Or Before 06-Jan-2022 that describes this very issue. Ford essentially notes that pad material will become dislodged and get attached to the rotor. That they're not replacing this under warranty and that they haven't issued a recall for brake pads they've already redesigned because they can break apart is extremely dangerous. There's notes of this happening to cars on Mach-e message boards and Ford is aware because they've issued a TSB: [XXX] (for example). When stopped at a stoplight, in unbridled mode with one pedal driving engaged, the car surged forward on it's own. The surge forward caused my vehicle to rear-end another vehicle. I would assume the failure was the one pedal driving system and it is available for inspection. The problem has not been reproduced or inspected yet, but we have an appointment for a Ford dealer to evaluate the car. The only warning was the pre collision warning which did go off.

My car has repeatedly had to have rotors replaced due to warping. Between 12,000 and 36,000 miles I’ve had 7 pairs of rotors replaced. After exceeding 50 mph, when braking I decided violent steering wheel vibrations between 43-35 MPH when braking normally. I have service records and video to back up my claim. Ford is calling this “normal wear and tear” however no other 2021 mach-es vehicles in my geographic area have had this issue, in fact none have had to have rotors replaced at all. This is a serious safety issue, occurring approximately every 3,000 miles. This points to a defective vehicle, Ford refuses to cover the repairs now that the vehicle has passed 36,000 miles despite having a verified service record of 7 rotor replacements. The date below is for the most recent issue, it has been going on for a year and a half. Ford clearly doesn’t understand how to fix it, as the same fix has been recommended every time to the same result. I can’t upload the videos but happy to send for additional support of my the defective vehicle assertion, it helps to show how much of a safety issue this is.

I am getting a warning in the car that the front camera is failing. The warning includes an error that the front camera failed, and that the forward collision is unavailable. Each time this happens, this causes the cruise control, lane centering and lane departure systems to disengage, which means the vehicles hard brakes at highway speeds when this happens. It disabled forward collision detection when this happens. Ford has TSB for this but when I took it in for repair/replacement they said it wasn't covered by anything and charged for the fix.

This work was completed along with numerous other software updates needed as indicated by dealership service department. Shortly after getting car back a "1Pedal Drive Fault - Press Brake Pedal To Reduce Speed" warning message was displayed on the instrument panel and the one pedal function quit working. This feature reset it self after the car was parked overnight. Yesterday while driving over many miles this same warning message appeared to happen again and the one pedal feature quit. This happened while vehicle was traveling in an active school zone very near a crosswalk being used by students. This could have been a very bad situation if not quickly going from one pedal braking to two pedal braking which is needed to stop the car.

One pedal drive system failure. Had the 22B50 charge port recall done on the 18th of April 24. After leaving the dealer upon approach to a light one mile away, one pedal disengaged during de-acceleration for the light. Warning did come up near the same time. It felt like vehicle was accelerating, but just loss regen. It has since happened daily. sometimes multiple times. Restart the car and it works normal. The recall was to reprogram the BCCM according to the dealer.

After getting the HVBJB recall, I have had 1 pedal drive mode turn off unexpectedly about once a day. Since I expect my car to start breaking when I am off the accelerator pedal, when 1 pedal drive disables the car will move faster than expected if I am off the accelerator pedal. This could result in running a stop sign or hitting another car due to this unexpected lack of breaking. I do get a warning (while driving) on the dash screen that 1 pedal drive had a fault and was disabled. I have not taken my car into a dealer.

Having completed the high voltage junction box recall with the Mach-E a week or so before, I was braking in order to complete a left-turn in one-pedal drive mode (therefore lifting my foot off the accelerator causing the vehicle to slow down). I received a notification as I did so that one pedal drive mode had a fault and was deactivated (this appeared on the screen by the steering wheel). This caused the car to lurch forwards, as it shifted from OPD to standard drive mode. Fortunately no cars were ahead of me and I was able to quickly slow the vehicle by braking, but I'm alarmed as to what would have happened had this been in traffic.

I drove the car to work with no issues. I left work a little over 9 hours later. I was able to enter car without issues. Turned on car and didn’t notice any issues. My phone connected as usual via CarPlay and I navigated home. Leaving the work area is low speed, so I never exceeded 20 mph and just went by feeling, but once I was on a more open stretch of road, I noticed that the instrument cluster was frozen and the speed it was displaying felt too slow compared to how I felt I was going. In essence, I couldn’t tell how fast I was going. After closer examination (mind you I was driving while figuring this stuff out), I noticed that the instrument cluster would update once in say 10seconds or so (but it was variable). I tried to set cruise control to initiate blue cruise, but I couldn’t tell what speed cruise control was being set to due to this enormous lag. I also wasn’t sure if the system would also exhibit this lag. Finally, I noticed that even the lights indicator for the blinker would refresh very slowly but the audible “click” associated with the blinker was normal. The drive home was very distracted and painful due to this issue. I documented the behavior in a video if needed.

heavy vibration and pulsation from brakes when stopping from 50 mph. Steering wheel jumped out of my hands on a panic stop one time. I would say this is a safety issue that apparently Ford already knows about with service bulletin #22-2023. Ford refuses to make the repair under warranty because they say it's a wear and tear issue-even thought they admit to faulty brake pads. The problem was confirmed by the dealership and they wanted $1300 to fix their defective brake pads.

After transitioning from i35 south, to Highway 183 towards Austin-Bergstrom International Airport, the auto-pilot (Ford Blue Cruse) fails, the speedometer fails, the battery level indicator fails, the instrument cluster fails, cruse control picks a random speed and struggles to keep it, lane keep assist is erratic at best. This occurs every time, and in both directions, I take my vehicle on that highway. I took a video of it this last time, and offered it to Ford. They recommended I file this report with this agency.

Significant vibration in the steering wheel when braking between 40-50 mph. It was reported to the dealer and "fixed" but just had it "fixed" for the 4th time. The vibration in the steering wheel is alarming and disconcerting to say the least. No warning lamps or other indications were present any of the times this has been experienced. After each "fix" the timeframe/mileage between occurrences has reduced. The last 3 times, the dealer had the car for over a week to allow them to perform a "comprehensive" fix. If you do a search on vehicle dashboards, this is an issue with many other 2021 mach-e vehicles. Ford continues to pretend like this is limited to my vehicle. While they have covered the reports thus far, having a vehicle with (now) 30,000 miles on it and multiple issues with the braking system (and it appears to be a systemic issue) is unsafe and causes significant lack of faith in my ability to brake safely. While I was assured by Ford that the brakes work fine and it wasn't unsafe, a jarring vibration in the steering wheel when decelerating in a specific speed range is not safe. The information in the "additional" details is for the latest (now 5th time) this is being addressed.

There is a software problem which was introduced in a previous OTA which causes drive modes to change. These drive modes are Whisper, Engage, and Unbridle. Each has their own acceleration/throttle response characteristics, as well as steering and suspension feel. You also have the ability to enable one-pedal drive in the drive mode menu. The software bug will randomly throw an error on the main screen that says "Drive Mode Not Available" change your drive mode from Whisper or Unbridle to Engage, affecting the throttle response that can occasionally lead to unintended acceleration. On occasion, it can also lead to disengaging of the one-pedal drive mode at the same time, which can cause the vehicle to not slow down when the foot is lifted off the accelerator. If the driver is unaware that 1PD has become disengaged unintentionally (which cannot be re-engaged without power-cycling the vehicle), it could lead to a crash as the driver is not expecting to use the brake pedal. In a moment's notice situation, it could be deadly. The vehicle has not been inspected, however, this problem has been reported by many other Mustang Mach-E owners. I will link my forum post at the end of this report which has other claims of this fault. The vehicle is available for inspection upon request. It cannot be easily reproduced as it is entirely random. Yesterday (11/8/2023 @ 1:40 EST), I was pulling into a parking spot with concrete posts around it. This time, it happened as I was turning into the spot. I was in Whisper mode this time. I noticed the car SPED UP a bit as I was letting my foot off the accelerator, and I had to slam on the brakes. Sure enough, "Drive Mode Not Available" was up on the screen. I was about 1 inch away from this concrete post. All 4 brake rotors warped at once. Had vehicle serviced for 10000 mile evaluation which included rotating tires and checking brake only 6 weeks earlier. Only drove car additional 200 miles since service. All rotors warped at once without excessive braking, heat or water.

Acceleration problem. The in this Mustang Mach E 2021 when I stepped on the 'gas' pedal there was no response then a very slow response, then a leap to a faster speed independent of my pressure on the pedal. I had to break to slow down. This was a constant presence until I was able to get home, about 2 miles away. I was terrified that I had no control over my speed, if I had to get out in front of traffic quickly or if I couldn't stop and my speed keep increasing. My concern is that this was a problem that lead to a recall issued 5/30/2022 and my vehicle was not included, although it met the criteria described in the recall. I think you missed me. My car was towed away on 9/16/23 to Middleton Ford where they can look at it 10/6/23.

The contact stated that while driving slowly in reverse and reversing of a parking space, the vehicle suddenly experienced unintended acceleration and erroneously drove across the street before crashing into an apartment building wall. During the crash, the driver sustained a lower back injury and left arm laceration which required medical treatment. The vehicle was towed to a local body shop. No police report was taken. The cause of the failure was not determined. The manufacturer was notified of the failure. The local dealer was not contacted. The failure mileage was 14,750.

The front camera system has failed. The cables need replacing, which was diagnosed by a Ford dealer. I have no collision assist, no cruise control, no lane keeping abilities, nothing that has to do with the camera in front. This is a known problem that Ford is not admitting to. All years of the Mach-E are having this issue, frequently. It has put myself and others on the road in danger by not being able to function as it should. Every time I turn the car on I get multiple warnings on the dash concerning these issues. This is not the only problem and safety issue I've had with this car in less than 9 months of owning it. Something must be done to hold Ford accountable.

I was driving on the highway (12) in Mandeville, Louisiana and the car (a 2021 all electric Mustang Mach E with only 13k miles) flashed a 'Stop care safely now' sign and then proceeded to power down within 3 seconds of the message. I exited (luckily I was at my exit) and felt the car lose power on the exit ramp. Three signals came up (battery - yet I was at 100% - then a turtle mode and not sure what the third symbol was) and then the car locked up - went into park, would not move or allow me to put into neutral to pull off a very busy highway ramp. I was forced to a dead stop on the ramp (very dangerous). I put on my hazard lights but cars still pilled up all around me, beeping etc. at high rates of speed. I called 911 and the police came out (took about 10 minutes during which time I had to direct traffic around me - again very dangerous) and asked that I move the car but the car would not move or even go into neutral. A DOTD tow truck towed me to a nearby Winn Dixie and then the roadside assistance towed to the Ford Dealer. The Ford Pass app send me the message attached below that said it was forcing a software update and the car would not be able to be used during this update - while the car was in drive and while I was driving it down a four lane highway! Very unsafe considering the car was connected to WIFI at home and that update could have been done at any time - not while driving. There was no notice prior to this that a software update was needed and we have it set for updates to happen at 1 am. The car relies upon software (not firmware) to run and if that fails, the car just locks up. This car should be recalled. I am lucky no one was injured including me trying to direct traffic on a busy high speed highway ramp. The policeman was very frustrated and made me get out of the car. Then the DOTD driver confirmed it would not budge. Thank God this didn't happen while I was in the middle of a 4 lane highway. It would have been like stopping on a train track.

All of the advanced safety features have stopped working in my Mach E The curse control does not work either the adaptive or the standard. The pre-collision system is not working, lane assit is not working. The absence of these working systems is a danger. Ford knows about this software problem with the software, however they refuse to fix the problem at there expense. The Mach E was designed from the ground up as software driven, the manufacture should be responsible to make sure that the software is up to date.

Stuck Disk Brake: The left rear disc brake was stuck. After releasing the parking brake the car would not move. After two attempts to reverse the brake released. There is a loud thumping sound from the rear wheel. While braking there is a violent vibration coming from the left rear wheel. The car is parked now and will be towed to a Ford Dealer. In the on-line Ford Mach-E forum there are other complaints about brakes being stuck with similar description as above. My car was last driven in wet weather and then parked in a garage. Temperatures at the time were cold but not freezing. The temperature at the time of incident was about 50 degrees Fahrenheit. This seems to be a safety issue.

I keep receiving a fault for the front camera which seems to be a very common occurrence amongst Mach-e owners lately. After receiving this message, the cruise control abruptly kicks off. All functions of the cruise control do not work in this instance…adaptive, speed keeping, lane control, etc. This fault happens about once every couple of minutes while driving. When I’m using cruise control in ‘one-pedal’ drive mode and this fault occurs, the vehicle immediately starts braking. This happened one time as I was entering a narrow bridge in Solomons, MD, and a car behind me nearly rear ended me.

The contact filed a claim with the insurance provider and the vehicle, and the home was repaired. The manufacturer was made aware of the failure and a case number was provided. The approximate failure mileage was 2,335.

The air bag deployed and impacted the driver in the face and she sustained knee injuries and cuts on her stomach and blood in her stool. A police report was filed. There were no reported fires. The vehicle was towed to an independent collision center. The vehicle was taken previously to the local dealer where the hardware was updated and a reboot was completed. The vehicle was repaired but the failure persisted. The manufacturer was not contacted. The failure mileage was unknown. The VIN was not available.

The rear brake pads have grinned into the rotors causing a rusted ring around the rear rotors. During braking, harsh metal-on-metal noise can be heard in low speed braking applications. Performance-wise, noticeably longer braking distances in low speed braking applications were observed. I Have been to the dealership multiple times however, I was told this was ‘normal’ by chief service managers. Loud braking noise and weakened braking performance should not be observed after fewer than 11k miles and less than a year of ownership.

When placed in drive the car takes off on its own without the acceleration pedal being touched. The passenger locks only work with key fob unlock button being pressed and the passenger side doesn't recognize the Fob for easy entry. The adaptive cruise control warning comes on constantly. The handas free parking never worked since i got the car in July 2021.

The contact stated while his wife was exiting the carwash and making a left turn with a passenger in the front passenger seat, she shifted from neutral into drive and depressed the accelerator pedal. The vehicle accelerated at high speed independently and crashed into the block wall. The air bags deployed. The contact's wife sought medical assistance and was informed that she had sustained a fractured rib on the left side of her body, pain on the shoulders, and pain in the rib cage. The passenger occupying the front passenger's seat sustained pain all over her body and a bruised rib and she sought medical assistance. A police report was filed. The vehicle was towed to a tow lot where it was totaled. The dealer was not notified of the failure. The manufacturer was notified of the failure. The contact became aware of NHTSA Campaign Number: 22V333000 (Vehicle Speed Control, Power Train), which he associated with the failure. The failure mileage was approximately 18,000.

On approximately March 20, 2022, I was driving my Mach E vehicle. I pulled into a driveway and parked my vehicle. Upon exiting my vehicle, my vehicle began accelerating forward up the inclined driveway. It then struck a parked vehicle and came to a stop. The car accelerated uninetentionally which is in alignment with the current recall. Additionally, the advanced emergency braking did not work as the brakes did not deploy and the car only went back to rest after I re-entered the car post-accident.

I was driving 70 mph on Hwy 99 in California about 35 miles north of Bakersfield when the car suddenly lost all power. A warning message appeared on the dash "stop safely now." I was able to coast over to the side of the road (there really wasn't much of a shoulder). It was extremely unsafe. The car would not move out of gear, would not start, etc. The car has 1550 miles on the odometer. I waited 1.5 hours for a tow truck to take the car to a Ford dealer. I was 4 hours from home. I am a 71 year old widow. I was traveling alone with my small dog. It was very frightening. The car has been at the dealership for a week. They have no idea what the problem is. The dealer is working with "a Ford engineer" to diagnose the problem. The Mustang Mach-E owners' forums are FILLED with complaints from other owners who are experiencing the exact same problem with this car.

"Considering a used Mach e in the 25k or less price I currently drive a subaru Crosstrek and my lease is coming up in December this year. I would love to get my first EV. I was initially considering another lease to a subaru uncharted (which hasn’t released yet) but looking at the used market, seems there are some good deals to be had for a used mach e. I’m in California so there is supposed to be a California version of the EV tax credit or rebate that replaces the federal one that Trump got rid of. But no word yet on when that will be available There’s also this 4000 rebate from Southern California Edison I can qualify for. I’m just curious what do people think about buying a used mach e? "Such a sad day So I’d like to present my very first photos with my 2021 Ford Mustang Mach-E Select AWD. I want to say that I only had the vehicle for maybe 3 months. Here’s what happened: I got a call from the Ford dealership (local) that they found a Mach-E for me around the price I was looking to pay. The only thing was that they wanted to make sure I was ready to put a down payment on it immediately because they had to purchase it from another dealership. Once I gave my okay, the same day they purchased the vehicle was the same day I put in my down payment and signed the paperwork and drove off with the vehicle.
