Vehicle: 2021 Ford Mustang Mach-E Component: Image Processing Module A (IPMA) / Front Camera / Forward Collision Avoidance System The IPMA and front camera system have failed, disabling Pre-Collision Assist, AEB, forward collision warning, and adaptive cruise control while driving. The vehicle is available for inspection upon request. Warning messages "Front Camera Fault Service Required" and "Pre-Collision Assist Not Available" first appeared August 24, 2022 and have recurred consistently over nearly four years. No accident, physical damage, or obstruction was present at any occurrence. This failure puts safety at risk by silently disabling AEB and forward collision warning during normal driving — systems a driver may depend on in an emergency braking situation with no advance notice they are unavailable. The problem was confirmed by a Ford dealership in February 2026. Their repair order documents: "RAN IPMA SELF TEST AND FOUND DTC SET U3000-49... HARDWARE TEST FAILED... FOUND TSB RELATED REC REPLACE MODULE." The dealer contacted Ford, who authorized 50% off the module replacement only, characterizing it as a "loyal customer discount" rather than a warranty or goodwill repair. I declined to pay for the fix. NHTSA Campaign 11028164 (superseding TSB 25-2156) covers this exact failure. Ford's own TSB acknowledges the defect and the dealer confirmed it applies to my vehicle, yet owners are being quoted up to $8,000 for a known safety system failure. The front camera has failed and it is available for inspection upon request. The safety of myself and everyone in my car is at risk when the pre-collision assist is not available due to this components failure. Yes, it is a daily occurrence and the camera never stays on for long. A fix was attempted once by a dealer. The component has not been inspected by either of those parties. Only the dealer whom the car was purchased from. Yes, warning messages pop up every time in the car and in the phone app about the issue. They first appeared before car purchase when the dealer fixed it in early February. Then it resumed again in late February/March.

An error message intermittently pops up on my vehicle that says, "Front Camera Fault", causing collision assistance and adaptive cruise control to suddenly stop working. This is a major safety issue, as a failure of adaptive cruise control while under operation may lead to a risk of an accident. Ford is aware of this issue and has issued TSB 23-2155, but they are treating it like a standard repair and not covering vehicles out of warranty. This should be a safety recall. Vehicle experienced immediate loss of motive power while in motion at 45 mph after "Stop Safely Now" alert. This is a repeat power failure. The vehicle was in service for 75+ days (Dec-Feb) for this exact defect. The Ford service center said they replaced the “battery controller” yet the issue recurred. The car previously flagged this alert while in motion 2 times prior (one on the highway going ~65mph and on another major street going roughly ~45 mph). As a Ford Reacquired Vehicle (Buyback), the failure to resolve this catastrophic electrical fault creates an extreme risk of crash and entrapment in traffic, putting myself, my family and the drivers in vicinity at significant safety risk. The car is currently at a different Ford Service Center. They have been able to reproduce the issue and are currently running diagnostics to identify a resolution.

Camera system that allows hands free driving, lane departure warning, collision warning and adaptive cruise control fails and all system offline. Extremely widespread on the 2021 models. Looks to be a cable route to the camera that was installed incorrectly. Went to dealership, they said they see this in almost all of the Mach E vehicles. Usually begins failing when hitting bumps -- system goes in/out. After a while, system drops out completely and all safety systems relying on them fail.

I was driving on the highway when the car gave me an error saying that the front camera is faulty and the collision warning system was giving an error. This stopped my cruise control and lane departure settings. I had just got my car back from Ford for this issue where they replaced a $7000 computer part and said it was fixed. This issue has been happening since I purchased the car in Dec 2024.

Car gave me a warning to bring car to dealer, who told me I need to replace the High Voltage Battery Junction Box. This part was recalled on other models, but not my specific model, despite the cars using the same part. Dealer said it will cost $3,593 to replace and Ford customer service says there is no recall for it. Also, I intermittenly have gotten a warning that the front camera will not work for forward collision warning or cruise control. It often happens after a bump in the road, but also happens at other random times.

Front camera fault. Pre collision systems don’t work without it. Large online community with the same issues but ford refuses to acknowledge it’s an issue. They won’t help even if errors start happening before warranty is over. And once warranty is over they charge large amounts to fix. The front camera system stopped working, so I lost access to Pre-Collision Assist—(e.g., Collision Warning, Automatic Emergency Braking) as well as all other driver assistance feature. This would occur suddenly while I was driving (sometimes at highway speeds) where I'd have to swerve during a turn while using hands-free driving. Every time I start the car now, I have a permanent error message telling me the safety systems aren’t available. I’ve tried restarting the vehicle and checking for any obvious problems, but nothing has worked. The dealer told me it’s a hardware problem with the camera or its wiring, but fixing it would cost more than I can afford ($4K+). Since Pre-Collision Assist is a key safety feature, not having it working puts me—and possibly other drivers—at greater risk of an accident. This seems to be a common issue with Mustang Mach-Es, and I’m worried about the safety impact. I hope NHTSA can look into this and see if a recall or some other action is needed.

Messages are: Front Camera Fault Service Required; Pre-Collision Assist Not Available; Lane Keeping System Fault Service Required. Ford is unable to correct these making it potentially unsafe. Additionally, they are not covering the attempt to fix this under the extended warranty. There are hundreds (if not thousands) of owners of the same model and year of my car, and subsequent years as well, having this issue with no resolution. There needs to be a recall.

There is a repeated front camera fault that prevents the "Pre-Collision Assist" from working, which could lead to a car accident and possible injury/fatality. It has been to the dealership many times over more than 2 years, and the issue still has not been resolved. The Ford dealer had been able to confirm and reproduce the issue. There is no consistency for when the alert appears on the car that the "Pre-Collision Assist" is not working, but it happens often.

Ever since we bought the car in mid-2023, we have been getting "Front camera fault" and "Pre-Collision Assist not available" error messages, at first intermittent and now constant. The errors pop up every time we turn on the car, and sometimes while driving as well, which is distracting and annoying. In addition, the cruise control doesn't work at all, and these front camera and pre-collision assist functions (which are safety features of the car) do not work. We brought the vehicle to two dealers for this same problem well within the warranty period and neither was able to fix it. We have tried to get these errors fixed on multiple occasions with no luck so far. Hansel Ford of Petaluma has our car and is now telling us it will cost $2806.21 to replace the IPMA module to fix the problem, and that it is not covered under warranty. Ford has a TSB addressing the issue: TSB 23-2155. I have spoken with Ford Customer Care on multiple occasions asking for assistance and they have failed to provide any help. This issue is widely reported on Mach-E forums as a known and frequent safety issue.

The forward collision detection system failed first. Then the car suddenly would not move and gave a pull over immediately warning on the dash and also flashed a battery light. It was 15 degrees outside and my wife and son were stuck in the cold. Not a safe environment. This is inexcusable for a car to be driving fine on minute and to be completely inoperative the next.

Backing up from a parking spot it failed to give warning of pedestrians crossing. It also fails warning for lane departures or Collision Alert warnings. All settings are on the console settings for drivers assist. Warning are given too late for pedestrians and Auto crossing while backing up. Forward collision warning does not work at all.

Front camera fault. Pre collision systems don’t work without it. Large online community with the same issues but ford refuses to acknowledge it’s an issue. They won’t help even if errors start happening before warranty is over. And once warranty is over they charge large amounts to fix. There is a TSB out for the issue and it seems like a pinched coaxial cable. Our safety is at stake and the number of occurrences can’t not be random.

I frequently get front camera fault warnings from my car and then it disables the adaptive cruise control and the collision warning and assistance features. I have attempted to fix this and the Ford dealer held my car for about 14 days (also damaging it twice). They attempted to replace the module but could not get the new one to work and then opted to put the old module back in and apply a software update. There is a TSB for this issue to replace the coaxial cable that goes between the camera and the IPMA module. For some reason the dealer did not opt to replace this cable after attempting to replace the module. There are a large number of reports of this problem on Mach-E forums and Reddit and clearly a recall is necessary because dealers are not adhering to the TSB (perhaps to wait out warranties). They're also not providing loaner or rental cars as required as a way to discourage consumers from having their vehicles repaired (while keeping vehicles for a long period of time). A recall would resolve the fact that that this is a serious safety and quality issue and protect consumers who are out of warranty. The fact that I'm continuing to get these errors after two weeks of service is disturbing. I'm including a copy of the TSB and the detailed error message from the FordPass app.

Front Camera Fault & Pre-Collision Assist Not Available Errors COLLISION WARN MALFUNCTION, COLLISION WARN NOT AVAILABLE or COLLISION WARN DISPLAY FAULT. Over 30k miles in the car so the warranty is over for bumper to bumper coverage. Dealer said an update would fix the issue. It cost 2k. Paid. The error came back within a day. Dealer says the next option is to replace all the wiring for the cameras. Which would be another 2-3k.

For over a year I got this warning that said pre collision assist not available. And it would go away after a few minutes. A few weeks ago it never left. Now I have that error as well as adaptive cruise not available. I cannot use my cruise control. I just recently had another issue and paid over 3 thousand. Now ford wants to give me another quote for 3 thousand. I searched and I'm not the only one with issue. This should he a recall problem as we are all having same issue.

Front camera stopped working after a long period of working intermittently (started to happen after passing warranty at 30,000 miles). Without that camera many safety features are disabled, like a front collision warning, lane departure, cruise control with adoptive speed, emergency breaking. There is a permanent fault on the front camera of the vehicle, making it unavailable. This has permanently disabled the vehicle's pre-collision safety feature. The adaptive/normal cruise control is also unavailable because of this. Prior to the permanent issue, the fault would occur randomly and without warning, creating an extremely hazardous condition, especially while utilizing the vehicle's "one-pedal" feature. Under this condition (using adaptive/normal cruise control and the one-pedal function), the fault would occur abruptly and without warning, cruise control would fail and the vehicle would brake simultaneously, without driver action. Several times, while driving on the highway, this happened to me. I was fortunate enough to not get in an accident. The dealerships have been unwilling to address my safety concerns. Please see to it that this is resolved. I have seen many consumer forums with this same issue.

Received intermittent warnings that the front collision warning system, cruise control and front camera were not working. The problem I think began after having my roof/windshield removed/reglued under recall. It was intermittent and I could not tell if software updates were resolving the problem, but then the problem became permanent at which time my car was out of warranty. Which does not seem fair since the problem began during the warranty period,. likely as a result of the windshield recall.

While driving I get a camera alert and a collision avoidance system is not working. Then I have to reset the system by pushing a button on the steering wheel. The car has been in for this problem 3 times since 2022.Now the 3 year warranty is up and Ford won’t cover the repair. May dealership has been dealing with this issue on a lot of cars. I filed a lemon law suit. They are not going to repair the car or by it back. They to want to give a couple of thousand. But nobody can tell me what the cost will be to fix the car. When stopped at a stoplight, in unbridled mode with one pedal driving engaged, the car surged forward on it's own. The surge forward caused my vehicle to rear-end another vehicle. I would assume the failure was the one pedal driving system and it is available for inspection. The problem has not been reproduced or inspected yet, but we have an appointment for a Ford dealer to evaluate the car. The only warning was the pre collision warning which did go off.

Vehicle receives warning message that front camera and pre collision asst is unavailable after driving it for about an hour. Vehicle was serviced following the ford directive to make sure that the service to the vehicle following the warning was followed correctly. This didnt suffice the warning and the warning continues to show up daily on the vehicle. This is a safety feature that is causing errors and knocking out any safety feature for the operator of the vehicle.

When driving two errors occur randomly and repeatedly. 1) Front Camera Fault, and 2) Pre-collision Assist inoperative. If driving with using any of the ADAS features, the car immediately begins to slow rapidly although each time it has occurred there has been no obstacle present. In one instance, it was on a freeway at 75 mph speeds nearly causing a severe accident due to the unexpected rapid deceleration. Luckily this was at 10pm at night. Had it occurred during higher traffic volumes I can almost guarantee there would have been a multiple vehicle collision. Researching online, it appears that the manufacturer has issued a TSB 23-2155 acknowledging the problem exists, but they do not see this as a significant enough safety issue to warrant a recall. When the failure causes complete disconnection of all ADAS features and the vehicle to attempt unneeded, unexpected, and unpredictable evasive maneuvers, I believe this to be a significant safety issue. The warning and attempted braking by the vehicle occur on a daily basis.

Vehicle received "Front Camera Fault" and "Pre-Collision Assist Not Available" warnings multiple times. This is a known issue at Ford as they have a service bulletin (TSB 23-2155 [XXX] ) on it and is widespread. There are forum posts regarding this with 64 pages of people posting about it happening to them [XXX] ). When this happens, it disables Adaptive Cruise Control, Pre-Collision Assist, and Forward Collision Warning. If you happen to be using Adaptive Cruise Control or Ford's HANDS FREE BlueCruise, the hands free system immediately stops working. Pre-Collision Assistant was faulty and needed to be replaced, apparently there are multiple EVs having the same issue thus causing a safety hazard for vehicles expecting it to work. The warning lamp first came on two years ago for 30 seconds than it disappeared, and again last year however it stayed on so I took it to the dealership and they had collaborated the camera and the issue was resolved, however a month after that the issue came back for 30 seconds than disappeared. It didn't stop there, a week ago that same issue appeared and I took it to one dealership and they said that they had to replace the module which was currently on backorder, so I took it to a second dealership in which the technician updated all modules but was still showing the same issue so they replaced the module and the module was faulty so they had to order a new module which I am currently still waiting for the dealership to finish with the service.

I am getting a warning in the car that the front camera is failing. The warning includes an error that the front camera failed, and that the forward collision is unavailable. Each time this happens, this causes the cruise control, lane centering and lane departure systems to disengage, which means the vehicles hard brakes at highway speeds when this happens. It disabled forward collision detection when this happens. Ford has TSB for this but when I took it in for repair/replacement they said it wasn't covered by anything and charged for the fix.

This car has had multiple recalls and issues with the power train. It has affected my safety on multiple instances. The cars sensors went out die to the batter overheating and it was having trouble delivering power to the sensors and other parts of the vehicle. When the sensors went out, the car would malfunction and it would think we are about to crash so it would break on its own and do other weird technical things. When the battery overheats it also does not allow effect cooling of vehicle and I live in a state that is very hot. I have reported these issues to the dealer multiple times, but Ford has not put out a remedy for this issue yet.

Pre-collision assist not working, despite multiple trips to dealership for this problem. Starting about 6 months ago, the Mach-E would periodically alert that the "pre-collision assist" was not working as a result of a front-camera fault. This was indicate by a visual indication on the dash of the front camera fault followed by "pre-collision assist unavailable" Recently, since recall 23B50 was performed on the vehicle, the pre-collision assist feature seems to not be working at all now. It's a safety risk to not have adaptive cruise control and pre-collision assist/emergency braking be unavailable.

This is the second or third time this has happened with the most recent instance being most concerning as the speed was increasing with time. The 2021 Mustang Mach-E has repeatedly shown these "Pre-Collision Assist" and "Front Camera Fault" errors ever since we got it 6 months ago. The errors can come and go multiple times during any drive. For my wife it is just an annoyance since she never uses Adaptive Cruise. But for me or anyone else using Adaptive Cruise, it becomes a safety concern because this error suddenly cancels the cruise and being an EV it begins regenerative breaking and slowing down much faster than other drivers expect while its driver must react as quickly as possible to return to pedal control. It's my understanding that this is starting to happen on many modern Ford vehicles at around this age and dealership service centers have been given guidance on fixing it. Ours has been in for this service and the error is still occurring.

The contact owns a 2021 Ford Mustang Mach-E. The contact stated while making a left turn into a parking space, the vehicle collision sensor did not alert the contact that a vehicle was nearby. As a result, the contact crashed into another vehicle while attempting to park. During the impact, the lower passenger light was damaged. No injuries were reported. The vehicle was taken to the dealer where it was diagnosed that the camera feature on the vehicle was malfunctioning due to the changing of the original brand of tires. The dealer did not provide any parts that needed to be repaired or replaced. The vehicle was not repaired. The manufacturer was made aware of the failure and a case was opened. While getting my recalls repaired at the dealership, I warned them of an issue my car was having with its front camera. I was receiving a “Pre-Collision Assist Not Available” notification each time I started my car. Upon investigation, the issue was described to me by the repair technician as. an internal failure IPM-A module on the car. This results in pre-collision no longer working on the vehicle along with the car no longer being able to use cruise control. The car is unable to detect vehicles in front of it to warn of a possible collision.

"Considering a used Mach e in the 25k or less price I currently drive a subaru Crosstrek and my lease is coming up in December this year. I would love to get my first EV. I was initially considering another lease to a subaru uncharted (which hasn’t released yet) but looking at the used market, seems there are some good deals to be had for a used mach e. "Signed Lease for Ford Mustang Mach-E but No Delivery Due to Recall – Paying for a Car I Don’t Have. Hey everyone, hoping for some advice.
