while traveling at approximately 80 mph with Full Self-Driving (Supervised) engaged, my vehicle experienced a simultaneous failure of the Full Self-Driving computer and the Media Control Unit (MCU) touchscreen. Without any prior warning, the touchscreen went completely black, and the vehicle immediately executed an uncommanded, sharp steering maneuver into the right lane. It was very apparent that if I didn’t take over it was going to continue crossing highway lanes and go off road. I have around 45k miles specifically on FSD and never has it failed in such an extreme manner. This sudden steering input put myself and other motorists at extreme risk of a high-speed collision or roadway departure. I was forced to physically overpower the steering wheel to maintain lane integrity and prevent a crash at highway speeds. The failure appeared to be a total loss of the vehicle’s primary visualization and automated steering systems, leaving me with no digital feedback regarding speed or surrounding traffic while the car was still in motion. Prior to this event, there were no warning lamps or symptoms such as GPS drifting or screen lag. I have since attempted a hard reset and a full power cycle, but the critical nature of this uncommanded steering at 80 mph suggests a systemic defect in how FSD manages hardware or software failures. The vehicle and its internal data logs are available for inspection upon request. This incident has been documented in the Tesla App under a service request for a full log review.

I parked my Tesla vehicle in a friend’s driveway. When I returned and opened the driver’s door, I entered the vehicle and attempted to put it into Drive. At that point, the vehicle display went completely blank and the vehicle became inoperable. I then attempted to open the door from inside the vehicle, but the door would not open and I was unable to exit the car. I was effectively locked inside the vehicle. I contacted Tesla customer support and remained on the call for approximately 1.5 hours before receiving assistance. Tesla customer support confirmed that the issue was caused by failure of the 12-volt battery. At the time of the incident, the outside temperature was below 0 degrees centigrade. The incident occurred in Cedar Rapids, Iowa. There was no prior warning or notification from the vehicle or the Tesla mobile app indicating that the 12-volt battery was at risk of failure. The failure resulted in loss of vehicle function and inability to exit the vehicle, creating a serious safety concern.

Tesla's Full Self Driving (FSD) stopped my car on trolley tracks today, and would have stayed on the tracks for a minute if I hadn't taken control and driven off the tracks. This occurred in downtown San Diego, where the trolley (light rail) has many level street crossings. They are well marked. You are not supposed to enter the zone around the tracks until the vehicle ahead of you has moved far enough for you to be able to cross the tracks and get safely past them. I was using FSD. It drove my Tesla onto the tracks before the car ahead of me had given me room to get off the tracks. The line of traffic was stopped for a red light. I would not have been able to drive forward until the light changed, perhaps over a minute. So, I took over and manually drove the car into the adjacent lane, which luckily was empty, and off the tracks. If a train had come while FSD was still in control, this would have killed me, and perhaps other nearby people. There was no warning message or other indicator.

I was driving slowly at a parking lot of a supermarket and preparing to park on my right. When I saw a car on my left front backing up, I pressed on the brake pedal immediately and turned the wheel slightly to the right. All of a sudden, the Tesla accelerated on its own with a very strong force and high speed, with the wheel turned sharply on its own toward the right. The Tesla then run through the steel fence of the parking lot, flying past the sidewalk, and finally settled on top of a big rock on the ground (which is placed there to protect the supermarket building). All of these happened so fast with such a strong force and all in an unintended manner. I was not injured and luckily no other persons or vehicles were involved. However, the vehicle was totalled and there were damage of the supermarket property.

During raining multiple teslas including my tesla last week experienced hydroplaning lead to close ti fatal accidents I believe the car inappropriately used emergency breaking that lead to accident, i am not if that was due to the car seeing water/reflection ln the ground I was driving the speed limit and all cars were driving at the same speed 70-75 mile per hour) I suddenly started spinning with no warnings It is impossible that multiple teslas experienced the same thing more than other cars I am trying to get the data from tesla to confirm this, but it is not easy to make them get us what exactly happened I am seeking third party analysis hopefully once i get the data, but it would be best done from NHTSA The damage to my car was due to hitting the road and my car went to the middle due to hight difference No other cars or objects were hit If i hit anything, it could have been fatal

While driving on highway, my Tesla all of a sudden accelerated unintentionally and violently swiveled, not letting me take control of the steering wheel. My car spun 180 degrees and I had to drive to the grass on the side to stop the car. Autopilot was not on, cruise control was not on, I was driving normally. My entire family was in the car and they witnessed the situation. This was extremely scary and could have caused injuries or even death. This is the second time it happened to my Tesla (similar situation, my wife was driving that time).

My mother was involved in a fatal crash on August 30, 2022. We believe there was something unusual that occurred with the Tesla Model Y as it was on a quiet residential street and the car seems to have accelerated suddenly and she hit a pole. We are still waiting on the California Highway Patrol report.

Tesla full self driving drove off highway curb and dropped 10 feet down, vehicle was total loss, airbags popped. There was a car trying to change to Tesla lane, Tesla overacted and took over control, and driver was unable to take back the control while in full self driving mode, no warning sign whatsoever during the accident, Tesla just drove off high way curb by itself. No collision between Tesla and other car. As experienced driver for over 30 years, I have never had accidents at fault, never made such silly mistake to drive off high way.

In a rainy day I drove Tesla Model Y (AWD) on highway 1 in the Raleigh NC area. The highway 1 is under construction. There are only two lanes open with cement barriers on both sides. While I was driving on the left lane, the car suddenly lost control and slide to the right. I tried to hit break and turn left to avoid the SUV on the right lane, but my car end up turning 180 degree and driver side rear-end hit the cement barrier. Fortunately no injury and no other vehicles involved in the collation. It was raining but not hard. I don't understand why the AWD Tesla would suddenly lost control and slide to right. I can't remember if autopilot was on or off. I tried to request data from Tesla but was denied. Their email indicates only California registrant can request data. I believe the Teals driving and EDR data can really help to confirm if it was a weather issue or not. I have dashcam video ready to upload.

I was going on I-95 North (Palm Beach County) on Autopilot (and I had Full Self Driving enabled in my car). It was raining, and I was at moderate speed, I would say 45-50 mph. The car started spinning uncontrollably. By miracle nodoby hit me, the card corrected after what I believe were 2 spins and continue on the same direction as traffic. I believe, with the nerves of the moment, that about 45 to 60 seconds later, I put auto pilot again, not knowing what hapened. And the car, out of nothing, started spininng again. 3 times, and I hit the opposite wall of the highway. By another miracle, nobody hit me (rush hour, I-95 north, 820am)

Tesla has dog mode, camping mode, and climate on mode, which shows the interior temperature on the screen and in the app. The Telsa was in "dog mode" and displayed on the screen and app that the interior of the car was 66 degrees, but A/C was clearly not on and the interior of the car measured near 100 degrees. If I had not checked on the dogs in the car immediately, they would have died.

On 5/5/2021 my wife was driving in the Tesla Y taking her two grandkids to school and was stopped at a traffic light. The car was in Hold mode with a car in front of her and a car behind her. The car does not have Auto Pilot. All of a sudden the car started moving forward in an accelerated mode. My wife did not push the accelerator. The Tesla hit the car in front and then went into reverse and hit the car behind. Substantial damage to all three cars. My wife and my grandkids were terrified and everyone started screaming. My wife couldn’t release her seatbelt because it tightened during the crashes. She had to call for help. This happened at 7:30am. The San Jose police took a report 21-125-0191.

This placed me at risk of being stranded and potentially exposed to hazardous conditions, including loss of transportation in unsafe or uncontrolled environments. Or stopping while driving. --- **Has the problem been reproduced or confirmed by a dealer or independent service center?** The issue was immediately reported to the dealership, and they acknowledged the problem by agreeing to an exchange of the vehicle due to its defective condition. I did reset / restart the vehicle / soft / hard reset, software update. and This both message got disappear and then again after sometimes its keep coming back. So i did appointment to rocklin tesla center and they gave me cost estimate $5770. Thats way too much so i decline but they said those message might keep popping up because of LOW VOLTAGE DISTRIBUTION AND CONTROLLERS. So i accepted that service and paid $359.68. But i am still getting same both messages on my tesla screen. Sometimes it does comes and sometimes it does not. I did online research and people with same problem stating that because of TESLA's poor manufacturing steering wheel and some kind of faulty circuit board causing this issue. My car never been to any accident and its very low on milage. Please look for other tesla owner who has same problem and tesla has to fix this problem without charging this much amount. Thanks

I was entering the on-ramp to [XXX], from [XXX], to merge onto the freeway east bound towards [XXX], when my Tesla suddenly lost all power, the brakes did not work and the power steering completely failed making it feel like it was locked up. I was traveling approximately 40-50 mph at the time nearing the gore point to merge onto the freeway. Fortunately I was in the inside lane and I rolled to a stop away from other vehicles. I could not steer at all. Had I been completely on the freeway, I could have been rear ended badly. I had my two young boys in the vehicle with me, both [XXX] and [XXX]. We were completely trapped in the vehicle with no power. My [XXX] old remembered to use the manual latch to get out. I opened my door manually and It broke my driver side glass window. Vehicles were passing us rapidly to enter the freeway. I called my husband who is a police officer and he came and picked us up. We called Tesla who said they could not get a read on our battery whatsoever. We called a tow company and had this vehicle towed to Tesla repair center in [XXX], after reporting our traumatic event. They claim our warranty is expired, but our vehicle only has 37,638 miles and should be covered under the 8 year, 120,000 mile warranty that covers a major malfunction like this. I own a 2021 Tesla Model Y with approximately 46,500 miles. The vehicle is currently displaying battery error code BMS_a079, and the battery will only charge to approximately 30%, which severely limits the driving range and makes the vehicle unreliable for normal use. I reported this issue to Tesla on March 6, but the earliest service appointment available to me is March 17 at the Tesla service center in Fayetteville, Georgia. Because Tesla service centers are not widely available, I must travel to another city to have the vehicle inspected. The limited battery charge creates a difficult situation because I must determine how to get the vehicle safely to the service center with very restricted range. I may have to arrange towing or attempt to drive a vehicle that cannot fully charge. This issue has also created a hardship for my household. I have a toddler and normal daily responsibilities, including errands and transportation needs, but the vehicle cannot reliably be used due to the charging limitation. Being without dependable transportation for an extended period of time places families like mine in a very difficult position. This problem appears to be related to the vehicle’s battery management system and is not the result of driver misuse. Consumers should not be placed in situations where a major battery fault leaves them with limited transportation options while waiting extended periods for service, especially when service centers are not easily accessible.

The car suddenly reduced lane assistance and steering assistance functionality. After calling Tesla they said it is not a software issue. I checked the service mode and it indicated a malfunction in the steering assist motor, which apparently costs $3500 to replace. Online, I have found many complaints about this problem that are not resolved by software updates. In addition, there was a large number used steering assist motors for sale online which indicates a more wide spread problem. This is not normal for cars to have this failure prematurely and is a serious safety hazard. I read online that some had their steering EPAS get locked while driving on the highway and nearly destroyed their Teslas

On Monday 12/29/2025, while driving my 2021 Tesla Model Y on the highway at approximately 70 mph returning from DTW Airport to my home with my children in the vehicle, the steering suddenly locked / the steering wheel would not turn. I was able to pull over to the shoulder. There were no warning lamps or warning messages displayed before or during the failure. After pulling over, I powered the vehicle off and back on. The steering partially worked again, and I was able to drive home. Later the same day at my friend’s house/driveway, the steering locked out again and this time it was completely locked, making the vehicle not safely drivable. Again, there were no warning messages or symptoms prior to the lockout. Tesla Service provided a written estimate documenting the complaint “steering locked out / cannot drive” and proposed replacing the steering gear/steering rack (“Steering Gear LHD,” part number 1188832-00-B) plus alignment, total about $3,127.70, at 96,042 miles. The vehicle and component are available for inspection upon request (Tesla can retain the replaced part). This is a safety defect concern because sudden loss of steering control on a highway without warning could lead to a crash, especially with passengers/children in the vehicle.

Both rear seat belt buckles (driver-side rear and center) fail to latch securely. The metal tongue inserts fully into the buckle but does not lock with an audible click and releases immediately upon light tension. This prevents the seat belts from restraining passengers in a crash. - No visible debris, damage, or contamination in either buckle. - Issue occurs consistently on multiple attempts and with different seat belt tongues. - Vehicle is 2021 Model Y Long Range with 83,610 miles. - **Problem first noticed yesterday (November 2, 2025)**. - Tesla Service has refused repair under warranty, stating the vehicle is out of warranty, despite this being a critical safety component governed by FMVSS 209. On October 2, my Model Y battery was replaced with a defective cell. Tesla service center replaced the battery. The vehicle could not charge or operate, making it unable to be driven. Today, November 2, while driving my car, my Tesla ABS system and other battery components experienced total failure while driving. For a brief period of time, I was unable to brake completely and my vehicle felt like it was skidding. At the time this occurred, my 1 year old daughter and pregnant wife were in the vehicle. The main vehicle computer (MCU) failed. The problem first appeared in May 2024 with intermittent rear camera malfunctions. It has since worsened. Most recently (08/28/25), the center display went completely black and the vehicle became inoperable — I could not start or move it. This left me stranded in a dangerous neighborhood late at night and required the car to be towed. When the display fails, I lose access to the rear camera, climate controls (including defrost), and turn signal indicators. These are safety-critical systems, and in this case the failure also disabled the ability to drive the vehicle entirely. The problem has been reported to Tesla. They inspected the vehicle but claimed the earlier camera malfunction was unrelated to the current computer failure. No independent mechanic has inspected it. The failures are ongoing and reproducible. No warning lights or alerts appeared before the system shut down. The vehicle is available for inspection upon request. Being that this issue originally arose back in May, 2024- it should fall under Tesla’s warranty which expired in December 2024.

Brake line failed at the left rear tire. Corroded line. The line is corroded on the right rear also. These are my observations. The brakes failed when driving locally at lower speeds so the car was able to eventually stop due to the regenerative braking. Will be towed to the dealer for inspection/repair. Inspection/repair will occur on Sept.15, 2025. No warning until after the failure.

I’ve had to replace the steering rack, upper and lower control arm for the left wheel, entire computer motherboard. I drove the car for less than 70k miles and zero accidents and zero modifications. Yes I am open for inspection. While driving with fsd lane keep failed causing the car to veer to other lane. The tesla repair shop said the steering rack was sending errors and the whole rack needs to be replaced. They said it was both electrical and mechanical failure. Previously I’ve had the control arms replaced because it was making clicking noise when I turn the wheel. They said it was due to front compliance link and lateral link issues. Also had entire computer board replaced previously. The error message after the incident was lane assist disabled, steering assist reduced. In the maintenance mode it said “ The drive interface (DI) or drive inverter (DIx) electronic control unit (ECU) detects a low voltage (LV) battery undervoltage event.”

I needed extreme efforts to push the steering wheel to go off highway to avoid accident. This happened on 6/14/2025 around 3PM. Tesla service diagnosed "internal electrical/mechanical steering rack failure", but no recall was issued for my VIN. I've reviewed NHTSA Recall SB-25-00-004 (2025) for 2023 Model Y involving EPAS/steering assist failure. The symptoms in my car appear identical. I believe the issue may affect more model years than the recall currently covers and request HNTSA investigate further.

“Front passenger Safety restraint system fault, service required” RCM2_a636 message keeps appearing on the screen. It happened right after I bought the car, so I took it to Tesla to fix. It went away for a few months and now it’s back again. According to Tesla, this fault, “ Based on the airbag system's self-diagnostic checks, a persistent fault has been detected in one or more safety restraint components. This could involve seat belts, impact sensors, occupant sensors, or related safety components. • If the airbag warning indicator stays on, the vehicle should not be driven until inspected. • Recurring faults require immediate Tesla service attention. • Pre-tensioner or sensor damage can compromise proper seat belt functionality.” This is a serious safety issue that should be addressed in a recall as it could jeopardize occupant safety of the seat belts or air bags do not work. It also appears to be somewhat widespread and persistent. Tesla also does not appear to have a permanent fix as the first time I took it in they made a comment about one repair already having been done on this before I got the car as a certified used one from Tesla.

"I Test Drove a Model Y Today - WOW I test drove the Tesla Model Y today. I had never been in a Tesla previously. I was floored by this car. I'm sold. Regenerative braking is interesting. I was trying to explain to friends my driving experience. So there is a master brake pedal but you don't use it much. If you ever played the old school RC cars, where you pull the trigger and the car zooms off, and you ease off on the trigger and the car slows, that's exactly how the Tesla drives. You have a zoom pedal! And if you ease off on the zoom pedal you slow rather quickly and will come to a stop. The concept took me a few minutes because it's so foreign to how I learned to drive 30+ years ago. Very fun after you learn the perfect touch on the zoom pedal! Speaking of zoom. Much to my SOs chagrin I hit the freeway with the intention of using Autopilot. I let the car drive itself for a couple of minutes. Perfectly navigated down I80 at 75mph. Felt solid. Lastly my SO and I came away with the same impression on fit and finish and overall build quality. After reading many horror stories over the years about Tesla's quality, this car was flawless in and out. Very upscale feel inside. Quiet and comfortable. Really happy with the car in that respect as well. The TouchPad was very intuitive. I ordered a Red LR. Still undecided on FSD. I was happy with the performance of Auto Pilot. "2021 Model Ys - Might have problems opening the door. The Office of Defects Investigation (ODI) has received nine Vehicle Owners Questionnaires (VOQs) reporting an inability to open doors on Model Year (MY) 2021 Tesla Model Y vehicles. The most commonly reported scenarios involved parents exiting the vehicle after a drive cycle in order to remove a child from the back seat or placing a child in the back seat before starting a drive cycle. In those events, the parents were unable to reopen a door to regain access to the vehicle. Four of these VOQs reported resorting to breaking a window to regain entry into the vehicle. Although Tesla vehicles have manual door releases inside of the cabin, in these situations, a child may not be able to access or operate the releases even if the vehicle’s driver is aware of them. As a result, in these instances, an occupant who remains inside a vehicle in this condition may be unable to be rapidly retrieved by persons outside of the vehicle. Entrapment in a vehicle is particularly concerning in emergency situations, such as when children are entrapped in a hot vehicle. For awareness, NHTSA has a Child Heatstroke Campaign that highlights the dangers to children entrapped in hot vehicles. Based on ODI’s preliminary review, this condition appears to occur when the electronic door locks receive insufficient voltage from the vehicle. Available repair invoices report replacement of the vehicle’s low voltage battery after the incident. However, no VOQs reported seeing a low voltage battery warning prior to the exterior door handles becoming inoperative. The Tesla Owner’s Manual “Jump Starting” and “Opening the Hood with No Power” sections describe a multi-step process for restoring power to the electronic door locks in order to enable their operation from outside of the vehicle. This process requires applying 12 volts DC from a separate power source to two different points accessible from the vehicle’s exterior. The subject incidents suggest that this process may not be readily available to owners or well known. For instances where an occupant is able to access and operate interior door handles, the Tesla Owner’s Manual “Opening Doors with No Power” section identifies the manual door releases inside of the vehicle’s cabin. ODI is opening this Preliminary Evaluation (PE) to assess the scope and severity of this condition, including the risks that arise from the conditions reported in the VOQs. This investigation will also assess the approach used by Tesla to supply power to the door locks and the reliability of the applicable power supplies. At this time, NHTSA’s investigation is focused on the operability of the electronic door locks from outside of the vehicle as that circumstance is the only one in which there is no manual way to open the door. "The Tesla Model Y LR Brand new car continues to have issues after the software update after 3 attempts to fix it, and the loaner was denied :(. What would you do next? We picked up our brand-new Tesla at the end of December 2023. We were very excited as it was our first car. After testing multiple vehicles of different brands, we decided on getting the TESLA Model Y majorly because of its convenience features, i.e., Blind spot indicator, Side Camera, Backup Camera, Navigation, Autopilot, etc. Within 15 days of driving, we got a Sentry mode failure error on our Tesla app. Upon further investigation, we also found that the side cameras, backup cameras, and navigation were also not working. ​ **Attempt 1:** We immediately took it to the service center in Jan to see if they could help us fix the issue. The service center accepted all the problems and mentioned they needed time to fix them as it might be a computer or firmware update issue. They gave us a loaner and assured us to resolve the issue. Four days later, we got a message notification that the car was fixed, and when we went to pick it up, we noticed the cameras were blacking out intermittently(2-5) seconds. They again took the car, gave us the loaner, and mentioned they would send this to a high-caliber technician to fix it. **Attempt 2:** Fifteen days later, we again got the notification to pick up the car, as it had been fixed, but this time, while picking it up, we noticed water getting clogged at the trunk hatch. We brought this to the service help desk, and they again kept it at the service center as they needed time to fix it. **Attempt 3:** After a few days, for the third time, we got a notification that all issues were resolved, and we were finally excited, keeping our hopes high to drive back our car home happily. But again, lousy luck, as when we went to pick up the car and moved out of the parking lot, our vehicle was swamped with all of the issues we faced in January 2024. This time, again, no cameras on screen, navigation, autopilot, or sentry mode were working anymore. We immediately addressed it to the service center. They asked to wait patiently for the next update when the issues will fix themselves. They have no timeline for when we will get the new update. They had engineering work on the car multiple times and confirmed it was not a computer replacement and would not solve these as it was a firmware issue. The service helpdesk argued that these Cameras, Blind spot indicators, Side Cameras, Backup Cameras, Navigation, Autopilot, etc., are all convenient features, and the car is safe to drive to, which we disagree with as the decision to buy a Tesla car was significant because of these features which makes it safe to drive. Hey everyone, Just wanted to see if anyone else with a 2026 Tesla Model Y Long Range AWD Juniper is facing the same issues I am. I took delivery of my Juniper on July 11 2025, and by August 21 Tesla had to replace the entire computer because Autosteer completely stopped working. The service report said it was caused by a low-voltage power state issue in the Autopilot system. Then on October 9, they replaced the forward-facing Bi-Cam and the rear-view mirror triple-camera assembly and yet, here we are. I’m still getting • Autopilot degraded • “Autosteer temporarily unavailable” even on well-marked highways • Smart Summon hesitating or refusing to move • Autopark trying to park into curbs • Random camera degradation warnings that appear and vanish After talking with several Tesla technicians and reading through other owner reports, it’s clear this isn’t an isolated issue. Many Juniper owners are dealing with the same recurring failures. These aren’t minor calibration quirks; they’re serious reliability problems that make Autopilot borderline unusable. "Extended Warranty Refund Disaster Posting here hoping that someone may know what options I have left in order to get my refund for an extended warranty cancellation. I've tried everything I can think of, been patient (I think) and have no refund 4 months after taking delivery and trading in my model Y. If anyone knows managers within Tesla or anyone I can speak with that knows anything helpful about the extended warranty cancellation form process I'd really appreciate it. I feel like I'm taking crazy pills. **Background and timeline:** Owner of a 2021 Tesla Model Y and bought the $2,000 extended warranty through the app. In July 2024 Tesla came out with their 1.99% financing and ability to transfer FSD to a new car. I qualified for the Federal Tax Credit along with the New Jersey tax credit so the monthly payment would be about $200 less if I traded in my 2021 to finance a 2024 model Y. Before I made the purchase I messaged with Tesla since the mileage of my trade-in would barely go into the coverage (coverage starts at 50K miles and I was around 51K). I wasn't going to move forward with the new purchase if I'd essentially be losing almost $2K. The tesla representative told me that I would be able to get a prorated refund for the amount of the warranty based on my mileage. All I needed to do was fill out a one-page form that they provided and give it to the service center when I took delivery of my new model Y. Seems simple, right? I thought it would be, but instead it has been a nightmare and it seems there is no accountability for the people at Tesla that handle this process. On 7/31/24 I took delivery of my new model Y and gave the signed form to the tesla representative that organized my delivery. The representative clearly did not know what the form was and he had to speak with other employees about how to handle it (this was actually the longest part of the delivery process). He eventually spoke with someone that knew what the form was, accepted it and I was driving off in my new model Y. I expected that I'd hear from Tesla soon about when I would be getting the refund. Hearing nothing 2 weeks later I followed up with the service center on 8/15/24. I heard back promptly the next day on 8/16/24 and they asked for proof of purchase of the warranty and if they had that then they could process the refund (how they don't already have this information since it is linked to my account/the trade-in car I have no idea). I gave it to them 2 hours later and was reassured that this is all they needed to process the refund. Apparently that wasn't accurate because as of today, 11/27/24, I have no refund or idea when to expect it. Follow up on 9/3 (no response) and again on 9/12. I was told on 9/13 that the "finance team that processes these requests" resubmitted it (no explanation as to why it had to be resubmitted) and that I "should see a payment in the form of a mailed check within 3-4 weeks." I was trying to be patient, but on 10/25 (6 weeks"
